CAPA without the spreadsheet: make the action the record
Management systems rot when the controlled document and the real work diverge. The software has to notice.
Nonconformances that cannot become work, training, or a document revision are just a list. A useful QMS is a router: NCR-441 should be able to spawn CAPA-88 and a procedure bump without retyping the asset.
The spreadsheet is a symptom
People keep CAPA in Excel because the “system” cannot carry an action across domains. The finding lives in quality. The work order lives in maintenance. The procedure lives in a drive. Each close-out is a copy-paste, and the copies drift. An auditor will find the drift before the dashboard does.
Notice the divergence
The software’s job is not to store PDFs. It is to notice when the controlled document and the work no longer match: a procedure revision that no job has used, a CAPA past due with no labour, a training that expired after the last audit. That is the signal. The spreadsheet cannot raise it.
